State of Colorado Health Form. Only Parts A, B, and C of this form are requiredCamp Alexander (Camp A), so please complete this health information for the scout and adult if camping, which you must bring to Camp A for check in processing. Page 1 provides detailed instructions. Please also note that this form includes a health check and must be signed by your licensed health care provider, so give yourself plenty of lead time to get this completed.

If the scout is attending a high-adventure BSA event such as Philmont Scout Ranch, please also complete Part D of this form.

If the scout requires a Recreational Scuba Diver’s Physical Examination, additional sections are to be completed including a Medical Statement provided by PADI, which is on this form.
